Duties of the Social Worker include: Conduct thorough assessments of patients' social, emotional, and practical needs, taking into account their individual circumstances and preferences. Develop person-centered care plans in collaboration with patients, families, and other healthcare professionals, ensuring that interventions are tailored to meet the unique needs of each individual. Provide advocacy and support to patients and families, helping them navigate complex healthcare systems and access relevant services and resources. Work collaboratively with multidisciplinary teams, including medical staff, nurses, therapists, and community agencies, to ensure coordinated and integrated care delivery. Facilitate discharge planning and ensure appropriate arrangements are in place for patients to transition safely from hospital to community-based care settings. Provide crisis intervention and support to patients and families experiencing acute psychosocial or emotional distress. Maintain accurate and up-to-date records of assessments, care plans, and interventions, adhering to professional standards and legal requirements. Requirements of the Social Worker include: Possess a Social Work Degree or its equivalent. Maintain current registration with Social Work England (SWE), showcasing your commitment to professional standards and ethical practice. Significant Adult Social Worker experience Previous Hospital experience Demonstrate a comprehensive understanding of UK legislation pertinent to adults, ensuring adherence to legal frameworks in your professional endeavours.
